Run a Take 5 using the SLAM framework -- Stop, Look, Assess, Manage. Guided risk assessment on any device, with smart escalation to JSA or SWMS when needed.
The Take 5 follows the SLAM method: Stop to think about the task, Look for hazards from a list of 21 pre-defined categories (plus an Other option), Assess who could be harmed and the level of risk, then Manage by applying auto-suggested controls.
Each Take 5 is assessed at a Low, Medium, or High risk level. Medium risk prompts you to complete a JSA. High risk requires a full SWMS. The system auto-detects hazards like heights, confined spaces, and electrical work that trigger SWMS requirements, and prompts for permits when hot work, excavation, or electrical tasks are involved.
A JSA is recommended before proceeding. Working at heights and moving plant identified.
Before completing, workers confirm four declarations: that they are fit for work, have the skills and training required, know the emergency contacts, and will stop work if conditions become unsafe. An on-screen signature locks in the assessment. Save as draft to finish later, or complete it on the spot.
Generate a professional A4 PDF with your company logo and branding. All four SLAM sections are formatted clearly, with the worker signature embedded. Share with supervisors, attach to job records, or store for audits.
